MISSION
The mission is to “empower the Nashville area to be active and eat healthy”.

MEETINGS 
The Community Health and Wellness Team meets the second Tuesday of each month at East Park Regional Community Center. Members find serving on this team provides opportunity for personal and professional growth, networking, and opportunity to be involved in community policy change and community service.
 
 The Community Health and Wellness Team seeks to reduce the risk of disease and early death in the community by raising awareness of and encouraging participation in physical activity and healthy eating. The Metro Public Health Department is the lead agency for the “Nashville Community Health and Wellness Team”, a community coalition for which the Health Promotion division provides coordination and support. The coalition is made up of representatives from hospitals, agencies, businesses, advocacy groups, and concerned citizens. During 2003, the team was incorporated as a not-for-profit charitable organization, and has been certified as 501(c)(3) by the Internal Revenue Service. This means that all charitable contributions toward the team’s work are tax deductible. The team is eligible to apply for grants from foundations which require 501(c)(3) status.
